Each One Matters
Mental health support for people living in residential aged care
The Each One Matters service provides tailored psychological support to residents in Residential Aged Care Facilities (RACFs).
The program focuses on early intervention and support for mild to moderate mental health conditions, using a place-based, GP-led approach.
Our goal is to improve mental health outcomes for older adults by delivering compassionate, evidence-based care that is culturally safe and integrated with existing supports.
What To Expect
Tier 1: Education sessions for RACF staff and/or families of residents.
Tier 2: Group therapeutic support, including peer connection and skills building.
Tier 3: One-on-one therapeutic support tailored to individual mental health needs.
Tiered Levels of Care
Individual support sessions using evidence-based approaches.
Group counselling to support transitions into aged care.
Psychoeducation and mental health literacy training for RACF staff.
Stakeholder engagement to foster collaboration between residents, carers, and facility staff.
Integration with natural supports such as families and loved ones.
