Clinical Care Coordination for Children and Young People
Navigating mental health challenges can be tough—for children, young people, and their families. That’s why Better Connect offers Clinical Care Coordination (CCC) for children and young people aged 5 to 25 years who are experiencing, or at risk of, mental illness.
Whether you’re a parent, carer, or young person seeking help, our team is here to guide you through the process and connect you with the right support in your community.
What To Expect
Our Clinical Care Coordinators provide:
Community Connection
Help linking with local mental health and wellbeing services.
Brief Intervention and Psychoeducation
Support when local services aren’t appropriate or available.
Collaboration with families, schools, and other key people in a young person’s life.
Integrated Support
For Parents and Carers
If you are a parent or carer of a child requiring Clinical Care Coordination, you will need to provide consent for your child to access the service. Our Clinical Care Coordinators will contact you to set up appointments with you and/or your child, and they will work with you to support your child.
We can also discuss any supports you might be able to utilise from our other Better Connect partners or other services.
How to access support
Need support? Contact Medicare Mental Health on 1800 595 212. They’ll discuss your (or your child’s) needs and connect you with the right support services, which may include Better Connect.
