Clinical Care Coordination for Adults with Mental Illness

Everyone deserves compassionate, coordinated care that supports their mental health journey. Our Clinical Care Coordination service is designed for adults living with severe mental illness who are being managed in a primary care setting. We aim to create a supportive environment where you feel empowered, involved, and respected throughout your recovery.

Our approach is comprehensive and person-centred, addressing both mental and physical health needs through collaboration with a network of professionals including psychologists, social workers, and mental health nurses.

What To Expect

Initial Assessment

A thorough clinical assessment to understand your mental and physical health needs.

Developed collaboratively with you, incorporating evidence-based therapies such as cognitive behavioural therapy (CBT) and acceptance and commitment therapy (ACT).

Personalised Care Plan

Coordinated Care

Integration with healthcare providers, family support, and community services to ensure holistic care.

Ongoing Support and Review

Regular follow-ups and adjustments to your care plan based on your progress and evolving needs.

Transition to Tertiary Care

If needed, we’ll support a smooth transition to more intensive services.

We continuously evaluate and improve our services to ensure the best outcomes for you.
